It's not surprise to anyone that madness is normal for Dorohedoro, but the episode manages to notch it up a bit by combining the usual frenetic and darkly humorous action with newly introduced (in the season at least) thrilling horror scenes. About a third of the episode is nothing short of a supernatural slasher film of sorts. The show also keeps solidifying itself as the most brutal anime of the year so far, which is not a minor feat, considering Jigokuraku 2 and the Hokuto no Ken remake are its direct competition in that regard.
That is not to say that the emotional substance of the series was lacking during the episode, as we get more backstory of side characters like the doctor Kasukabe, Ai, and more importantly, Ebisu. And also, the protagonists share a moment, of what nature? Well, find out!
