The episode begins with a distressed call from Chie to Osamu, her words indicating a deep fear for Kayako. Mob, ever the pragmatist, dismisses her concerns initially, but quickly shares his own unsettling knowledge of the Ebisu Mori family. He recounts his early life, thrown out for seeing the 'true form' of the miko, a stark past that shaped his views. Mob reveals the family's secret: a bloodline of spiritual daughters, implying Kayako's connection. His cold calculation suggests Kayako herself might be a manifestation of something evil.

At the kindergarten gate, Kayako meets Nana, who introduces herself as Kayako's aunt and a Miko from the main Ebisu Mori family. Chie is visibly shaken and distrustful of Nana's presence. Nana observes Kayako's low spirits, which Kayako later attributes to her inability to defeat a previous ghost. Feeling powerless and dejected, Kayako confides her sadness. Nana, understanding, gives Kayako an origami heart, telling her it is an amulet for when she must confront danger.

A seemingly innocent game of hide-and-seek begins with Kayako chosen as 'it.' However, the children's play soon takes a sinister turn. Sacchan and Makun are tempted by a 'good hiding spot' in a shed. As Sacchan enters, dark, swirling spiritual energy wraps around her, pulling her inside. Makun, watching this unfold, wears a wide, unnerving grin, suggesting his complicity or transformation. Kayako, oblivious, finishes her count, ready to search for her friends.

When Kayako finds Makun and Sacchan, they are disturbingly altered, their faces distorting as they call her 'weird.' Sacchan's face transforms into a grotesque, mocking spirit, taunting Kayako. Kayako, terrified, tries to run, then attempts to fight back, but Sacchan's spirit tells her she is 'useless' and can do nothing. Overwhelmed and defeated, Kayako sinks to the ground, angry and sad.

With Kayako isolated, the spirits of Makun and Sacchan turn their attention to Yuzuna, suggesting they continue playing without Kayako. Yuzuna's fear is palpable as Makun's distorted, menacing spirit form appears to her. Both Makun and Sacchan's spirit forms torment Yuzuna, their faces cracked and eyes blank, demonstrating the malevolent influence now possessing her friends.

Chie calls the children inside, but Kayako remains alone, consumed by sadness and fear. When other children approach her, they too distort into terrifying spectral forms, laughing menacingly. A spirit tells Kayako she is 'useless,' pushing her to the brink of despair. In a moment of hope, Kayako looks up at the sky, clutching the origami heart Nana gave her, recalling Nana's words about confronting what she must.

The spirits of Sacchan, Makun, and Yuzuna surround Kayako, their twisted forms taunting her, pulling Yuzuna into dark tendrils. Kayako, initially trapped, transforms; her eyes glow white with fierce determination. With a powerful lunge, she declares, 'Don't bully Kayako's friends!' and attacks, obliterating Sacchan's spirit with a burst of bright, spiritual energy.

The children return to their normal selves, startled and crying, expressing anger at Kayako for their game ending. Chie intervenes, relieved to find Kayako's energy restored despite the chaos. Kayako proudly presents the origami heart from Nana, an 'omamori' she says. Chie, curious, unfolds the heart and finds a disturbing message within, causing her expression to shift to shock and worry.

Mob observes Kayako from the bushes, taking photos with his analog camera, noting her renewed vigor. Osamu expresses unease, insisting Kayako cannot be left unsupervised due to the volatile nature of the Ebisu Mori family's secrets. Mob’s camera, however, captures a chilling truth: a shadowy figure watching Kayako, a spirit that moves only when unobserved. Osamu's inability to see this spirit directly leads to Mob being attacked and injured, highlighting a dangerous disparity in their spiritual perceptions.

Osamu bravely fights the spirits that emanate from Mob's camera, improvising with a hula hoop as a spiritual weapon. Despite his efforts, the spirits multiply, overwhelming him. Osamu, making a selfless decision, instructs Mob to flee while he distracts the entities, appearing ready for sacrifice. Just as dark tendrils pull Osamu, Kayako intervenes, wielding her own hula hoop, now a conduit for potent spiritual energy, to repel the spirits and save him.

Mob, overwhelmed with gratitude and relief, attempts to capture Kayako's display of power, but Osamu stops him. Osamu, his hand now covered in ominous purple spiritual marks, recognizes his current limitations and resolves to become stronger to protect Mob. Meanwhile, Chie presses Nana for answers about Kayako’s extraordinary abilities. Nana reveals the dark truth of the Ebisu Mori lineage, exposing their use of 'Curse Conception' to intentionally create spiritual mediums.

Nana further explains to Chie that the Ebisu Mori family's firstborn daughters are, by design, spiritual mediums,

Chie becomes increasingly distressed, recalling an old spiritual medium's mention of a 'spirit in Kayako's mother's womb.' The implication of a 'second child' born through 'Curse Conception' dawns on her. Nana opens the door to Mirai's hospital room, only to be met by a violent blast of spiritual energy, leaving her injured and bleeding from one eye. A man calls Nana, relaying the grim news of Mirai's recent death and her chilling last words: she wanted to 'surprise' her older sister with Kayako.

Wounded and with blood streaming from her eye, Nana confronts Mirai's hospital room, from which a torrent of spirits erupts, including Kayako's deceased brother, now also a spirit. Mirai's body lies twisted unnaturally, a horrifying testament to her dark machinations. Nana, clutching a bloody origami crane inscribed with black characters, is overwhelmed by the full, terrifying scope of Mirai's vengeful intentions. The episode closes on Nana's profound shock and terror, leaving a lingering sense of despair.
