In a ruined village, Diamond, a flamboyant lieutenant with purple hair, delights in the terror of his captives. He forces a young girl to support her father, who hangs by a noose, a gruesome game of endurance. Diamond proclaims that power is the only law, his laughter echoing the despair of the helpless villagers. This is sadism for sadism's sake, a spectacle of absolute control.

In a high-security prison, Club, with his long steel claws, toys with his 'livestock.' He offers a macabre chance at freedom, only to savagely kill a desperate prisoner who tries to touch him. Club revels in his power, proclaiming King's authority as he slashes at the cowering captives. Bat watches from above, his face twisted with disgust, as Kenshiro disappears, drawn by his unyielding sense of justice.

Kenshiro then disables Club's hands with the 'Goshi Retsudan,' shattering his weapons and striking vital pressure points. He gives Club sixty seconds to live, and in his terror, Club reveals King's identity and the location of Yuria. His body grotesquely collapses, his spine snapping as the 'Meimon' pressure point takes full effect.

Kenshiro and Bat move through the city, which feels more like a vast prison camp. High above, on a grand balcony, Shin, the man known as King, watches the chaos he has wrought. He is handsome, arrogant, adorned in finery, and utterly narcissistic, viewing Kenshiro's arrival as a mere opportunity to prove his superiority. His calm amusement amidst the suffering is a chilling revelation of his character.

City sirens wail, putting the gang on high alert, as Kenshiro and Bat hide. Kenshiro witnesses prisoners branded with Shin's 'Bloody Cross' symbol, a mark of ownership and humiliation. This ritual ignites Kenshiro's cold fury; he tells Bat this mark is a personal challenge, recalling how Shin betrayed their friendship, stole Yuria, and left him with seven scars. This isn't just justice; it is a long-simmering, deeply personal vendetta.

Ignoring Bat's pleas for caution, Kenshiro prepares for a direct assault on the palace. He sheds his outer clothing, revealing the seven scars on his chest, which faintly glow. His battle aura begins to manifest, a visible sign of his absolute resolve. Kenshiro instructs Bat to watch, declaring he will settle the score with the man who took everything from him.

In the opulent throne room, Shin sits beside Yuria, who stares out sadly, a queen in a gilded cage. Shin speaks to her with possessive affection, boasting of his power, but Yuria's heart clearly remains elsewhere. Shin reveals Kenshiro is tearing through the city, taunting Yuria with his intent to kill Kenshiro before her eyes, mentioning his final trump card: Heart. Elsewhere, Heart, a massive, calm man with a heart tattoo, chats politely with a terrified bartender, a surreal contrast to the city's chaos.

The bartender accidentally cuts Heart's hand, and a single drop of blood shatters his calm. Heart explodes into a berserker rage, his face a horrific mask of fury. He mindlessly slaughters everyone in the tavern, including his own gang, smashing through bodies and furniture. Amidst the carnage, he screams his hatred for blood and violence. Afterward, he regains his composure, wipes his hands, and calmly states his dislike for the sight of blood, preparing to intercept Kenshiro.

Heart, a colossal shadow, walks towards the palace gates, clearing a path of terrified gang members. Meanwhile, Kenshiro begins his direct assault on the palace stairs, his battle aura blazing blue. He carves through an army of guards with lethal grace, their bodies sent flying by the sheer force of his strikes. Bat watches in awe as Kenshiro disappears into the upper levels, his war cry echoing through the city, a promise of justice.

Kenshiro smashes into the throne room, facing Shin, who greets him with a taunting smirk. Yuria cries out, her eyes filling with life and relief at Kenshiro's appearance after so many years.
